Social Worker

We are seeking a compassionate and skilled Hospice Social Worker to join our team at LifeBridge Health. As a Hospice Social Worker, you will play a vital role in providing emotional and practical support to patients and their families during the dying process.
Key Responsibilities:- Assess patients' and families' psychosocial, environmental, and financial needs
- Develop and implement individualized plans of care in collaboration with patients, families, and other caregivers
- Provide counseling services to help patients and families cope with terminal illness
- Coordinate with community and agency homecare referrals for services
- Master's Degree in Social Work from an accredited school
- Minimum of three years of clinical experience, with one year in a community health setting preferred
- LCSW or LCPC licensure required in Maryland and Pennsylvania
