RN - Acute Care Case Manager

VNS Health is seeking a dedicated Registered Nurse (RN) to join our team as an Acute Care Case Manager. As a key member of our care management team, you will play a vital role in assessing member needs, coordinating care, and ensuring cost-effective healthcare services.
Job Responsibilities:- Conduct comprehensive reviews of service requests to ensure compliance with regulatory standards and organizational policies.
- Participate in case conferences with management to identify opportunities for alternative care options and provide input for department process improvement.
- Maintain accurate records of care management and provide telephonic case management services as needed.
- Assess, plan, facilitate, and advocate for member health needs, and coordinate with community providers for effective transitions and care delivery.
- Current RN license
- Certified Case Manager preferred
- Associate's Degree in Nursing required; Bachelor's or Master's degree in nursing preferred
- Minimum two years of experience in case management or acute inpatient hospital care
- Knowledge of Medicare and Medicaid regulations
- Working knowledge of Microsoft Excel, PowerPoint, and Word
