About the Role

VNS Health is seeking a skilled Registered Nurse to join our team as a Case Manager in Acute Care. As a key member of our healthcare team, you will play a vital role in assessing member needs, identifying solutions, and promoting high-quality, cost-effective healthcare services.

Key Responsibilities
  • Assessment and Planning: Conduct comprehensive reviews of member requests, including clinical record reviews and interviews with members, clinical staff, and medical providers.
  • Clinical Decision Making: Examine standards and criteria to ensure medical necessity and appropriateness of admissions, treatment, and lengths of stay.
  • Case Management: Coordinate with management, subject matter experts, physicians, and discharge planners to ensure timely and cost-effective care.
  • Communication: Deliver timely notification detailing clinical decisions and maintain accurate records of care management.
  • Collaboration: Participate in case conferences with management and identify opportunities for alternative care options.
Requirements
  • Education: Associate's Degree in Nursing or a Master's degree in Occupational Therapy required.
  • Experience: Minimum two years of experience with strong cost containment/case management background or two years acute inpatient hospital experience in chronic or complex care required.
  • Licenses and Certifications: Current license to practice as a Registered Professional Nurse in New York State required.
