About the Role

VNS Health is seeking a skilled Nurse Manager to lead our Acute Care Case Management team. As a key member of our organization, you will be responsible for overseeing the daily operations of care coordination, ensuring quality, compliance, and member outcomes.

Key Responsibilities
  • Lead a team of approximately 15 staff members in providing comprehensive home and community-based care services.
  • Collaborate across multiple departments to achieve quality care, compliance, and member outcomes.
  • Utilize software systems, including Excel dashboards, to manage processes, data, and performance metrics.
  • Coach team members on productivity and performance, fostering collaboration and decision-making.
  • Maintain a deep understanding of regulatory requirements and ensure adherence to policies and guidelines.
  • Develop and implement process improvement plans to enhance team efficiency and effectiveness.
Requirements
  • Current registration to practice as a Registered Professional Nurse in the state of New York.
  • Bachelor's Degree in nursing or Master's Degree in Social Work.
  • Minimum five years of experience in home care, care management, or a related field.
  • Minimum of two years of progressive experience in clinical management.
  • Demonstrated knowledge of NYS regulations regarding Medicare or Medicaid Managed Long Term Care health plans.
