Job Title: Oncology Nurse Navigator

Inova Health System is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as an Oncology Nurse Navigator. This is a unique opportunity to make a meaningful difference in the lives of patients and their families affected by cancer.

About the Role:

The Oncology Nurse Navigator will provide comprehensive care coordination and support to patients and their families throughout their cancer journey. This includes:

  • Assessing patients' needs and developing personalized care plans
  • Coordinating with healthcare providers and community resources to ensure seamless care delivery
  • Providing education and support to patients and families on cancer diagnosis, treatment options, and survivorship
  • Facilitating communication between patients, families, and healthcare teams
  • Identifying and addressing barriers to care and developing solutions to improve patient outcomes
Responsibilities:

The Oncology Nurse Navigator will be responsible for:

  • Conducting comprehensive assessments of patients' health and psychosocial needs
  • Developing and implementing individualized care plans in collaboration with healthcare providers and patients
  • Coordinating with multidisciplinary teams to address patient questions and concerns
  • Providing education and support to patients and families on cancer diagnosis, treatment options, and survivorship
  • Facilitating communication between patients, families, and healthcare teams
  • Identifying and addressing barriers to care and developing solutions to improve patient outcomes
Requirements:

To be successful in this role, the Oncology Nurse Navigator must have:

  • 3-5 years of progressive clinical nursing experience with primary care management of patient populations
  • Certification in oncology within 2 years of employment
  • Strong oncology knowledge and basic knowledge of managed care
  • Working knowledge of insurance coverage and procedures
  • Working knowledge of community resources
  • Understanding of healthcare facilities and processes
  • Proficient computer skills
