Job Title: Oncology Nurse Navigator

Inova Health System is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as an Oncology Nurse Navigator. This role is a critical part of our mission to provide world-class healthcare to every person in every community we serve.

Job Summary:

The Oncology Nurse Navigator will work closely with patients, families, and healthcare providers to ensure seamless care coordination and navigation through the healthcare system. This role requires strong clinical knowledge, excellent communication skills, and a passion for delivering patient-centered care.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' health and psychosocial needs, and develop individualized care plans in collaboration with healthcare providers and patients.
  • Coordinate care delivery, including scheduling appointments, referrals, and follow-up care, to ensure timely and effective treatment.
  • Provide education and support to patients and families, empowering them to make informed decisions about their care.
  • Collaborate with multidisciplinary teams to address patient questions and concerns, and promote timely follow-up on treatment and supportive care recommendations.
  • Facilitate individualized care within the context of functional status, cultural considerations, health literacy, psychosocial, reproductive/fertility, and spiritual needs for patients, families, and caregivers.
Requirements:
  • 3-5 years' progressive clinical nursing experience with primary care management of patient populations.
  • Certification in oncology within 2 years of employment.
  • Strong oncology knowledge, basic knowledge of managed care, working knowledge of insurance coverage and procedures, working knowledge of community resources, understanding of healthcare facilities and processes, and proficient computer skills.
