Job Summary

We are seeking a highly skilled Care Manager/Social Work/Social Services professional to join our team at Lakeland Care. As a key member of our interdisciplinary team, you will provide comprehensive social service support to our members, enhancing their quality of life and promoting their independence.

Your primary responsibility will be to coordinate and monitor the provision of services based on a comprehensive assessment of each member's identified outcomes and needs. You will work closely with our team to develop, coordinate, and evaluate member-centered plans, ensuring that our members receive the best possible care.

Key Responsibilities
  • Coordinate and participate in home visits and care conferences involving members, their supports, and providers.
  • Collaborate with our team to coordinate acute and primary care services, care transitions, and related follow-up care.
  • Conduct comprehensive, strengths-based assessments of members' outcomes, needs, and risks, and perform reassessments as conditions change.
  • Develop, coordinate, monitor, and evaluate member-centered plans, considering cost and effectiveness in authorizing services and choosing providers.
  • Implement risk mitigation strategies to promote members' health, safety, and independence while respecting their rights to appeal and grieve.
Requirements
  • Certified Social Worker in the State of Wisconsin with a minimum of one year of experience working with at least one of the Family Care target populations.
  • A four-year bachelor's degree or more advanced degree in Human Services or a related field with one year of experience working with at least one of the Family Care target populations.
  • A four-year bachelor's degree or more advanced degree in any other area than Human Services with a minimum of three years' experience working with at least one of the Family Care target populations.
  • Working knowledge of computers, computer programs, typing, and data entry.
  • Ability to access members' homes, which are not required to comply with ADA regulations.
  • Ability to multi-task and work in a fast-paced environment.
  • Ability to lift up to 25 pounds.
  • Current driver's license, acceptable driving record, and proof of adequate insurance.
