About the Role
The Full Time RN Case Manager is responsible for administering skilled nursing care to clients requiring professional services in their homes. As a key member of our interdisciplinary team, you will teach/supervise the family and other members of the nursing team while ensuring that all services are performed in accordance with physicians' orders and under the direction and supervision of the Clinical Manager and the Case Manager.

Key Responsibilities
- Report directly to the Administrator, Executive Director or Patient Care Manager Senior and maintain primary control and professional management of each patient.
- Act as the primary liaison between physician, patient/family and home health team.
- Provide direct patient care services to patients and supportive care to the patient's family with a reasonable case load while providing a trusting and compassionate relationship with your patients and their families.
- Manage and collaborate with an interdisciplinary group to evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
- Maintain all assigned patient clinical records and participate in company quality assessment and performance improvement programs.

About You
To be successful in this role, you must possess a RN license in the state of requested employment (or eligibility to obtain) and have at least 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology. We also look for candidates with flexibility and composure when supporting others in a time of crisis, as well as additional preferred experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing.
