Staff Pharmacist II

Under the Department of Pharmaceutical Services, the Cardiology Ambulatory Pharmacist provides pharmaceutical care to patients at UCSF Ambulatory Care Clinics. The ideal candidate will have a Doctor of Pharmacy Degree from an accredited college or university or equivalent experience, California Board of Pharmacy License or license eligible, and completion of a PGY1 Pharmacy Residency or equivalent experience.
Key Responsibilities- Obtain medication histories and review and assess medication profiles.
- Provide drug information to patients and health care providers, and collaborate with clinicians on drugs of choice and therapeutic plans.
- Ensure access to medications, educate patients on the proper use of medications, and ensure that patients understand cautions, adverse reactions, and interactions related to their therapy.
- Supervise pharmacy technicians to facilitate insurance authorizations for medications.
- Participate in an interprofessional team consisting of physicians, advanced practice providers, nurses, and other clinic staff.
- Doctor of Pharmacy Degree from an accredited college or university or equivalent experience.
- California Board of Pharmacy License or license eligible.
- Completion of a PGY1 Pharmacy Residency or equivalent experience.
- Excellent interpersonal skills and ability to communicate effectively with patients and healthcare providers.
- Computer skills to facilitate clinical patient data medication order entry.
