Job Description:

The Senior Diabetes Management Specialist - Clinical Educator serves as a key member of our interdisciplinary health team, responsible for providing expert diabetes education and self-management skills to patients of all ages. This role involves assessing patient needs, identifying barriers to learning, and developing personalized education plans to promote optimal glycemic management.

Key Responsibilities:
  • Develop and implement state-of-the-art diabetes education programs, incorporating the latest evidence-based interventions and technologies.
  • Collaborate with healthcare providers, nurses, and other healthcare professionals to ensure seamless patient care and effective glycemic management.
  • Conduct regular assessments to evaluate patient progress, identifying areas for improvement and adjusting education plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure comprehensive understanding of diabetes management and treatment plans.
  • Stay up-to-date with the latest research, guidelines, and best practices in diabetes care, applying this knowledge to inform education and treatment plans.
  • Participate in data collection and analysis to inform program evaluation and quality improvement initiatives.
Requirements:

To be successful in this role, you will need:

  • A Bachelor's degree in Dietetics or Food and Nutrition, with a post-baccalaureate internship resulting in registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD), or a current Registered Nurse (RN) licensure from the State of Nebraska or an approved compact state.
  • At least two years of experience in clinical practice, with a focus on diabetes management and education.
  • A Certified Diabetes Educator (CDE) certification, either at hire or within one year of hire date.
  • Basic Life Support (CPR) certification.
