Overview

Catholic Health Initiatives is committed to delivering exceptional patient care and improving the health of our communities. We are seeking a skilled Diabetes Educator RN or Dietitian to join our team.

This role is responsible for providing direct and indirect care to patients with diabetes, working collaboratively with the healthcare team to develop and implement individualized care plans. The ideal candidate will have a strong background in diabetes education, excellent communication skills, and a passion for delivering high-quality patient care.

Responsibilities
  • Conduct thorough assessments of patients with diabetes to identify their unique needs and develop personalized care plans.
  • Provide diabetes education and training to patients and their families, using evidence-based practices and established principles of teaching and learning.
  • Collaborate with the healthcare team to coordinate care and ensure seamless transitions between settings.
  • Participate in quality improvement initiatives to enhance patient outcomes and improve the overall quality of care.
Qualifications
  • Graduate of an accredited nursing school or dietetics program.
  • Current licensure as a registered nurse and/or dietitian in the state of Nebraska or Iowa.
  • Board certification as a Diabetes Educator or Advanced Diabetes Management with 39 months of hire.
