Job SummaryThis position contributes to the mission and vision of CHI Health by providing direct and indirect care to a designated group of patients across the age continuum. You'll work closely with our healthcare team to provide comprehensive care that addresses the spiritual, physiological, social, and psychological aspects of patient care.
Responsibilities- Conduct thorough individualized assessments of patients with or at risk for diabetes.
- Work with patients to identify mutually acceptable diabetes self-management training (DSMT) outcomes and behavioral objectives.
- Provide diabetes education according to individualized plans of care and desired outcomes.
- Develop self-management training plans to attain mutually defined outcomes.
- Coordinate and provide individual/group education sessions for diabetic outpatients.
- Provide leadership in maintaining and developing diabetic education program standards, policies, and procedures.
- Maintain ongoing evaluation of the diabetes education program.
- Assist in Quality Improvement activities related to patients with diabetes.
- Graduate of accredited nursing school.
- Bachelor's Degree in Nursing preferred.
- Licenses as a registered nurse and/or dietician in the state of Nebraska or Iowa.
- Be certified as a Diabetes Educator or Board Certified in Advanced Diabetes Management.
