Clinical Liaison Representative Job Description

Summary:

The Clinical Liaison Representative plays a critical role in facilitating the transition of patients from inpatient healthcare settings to post-acute services within the Mass General Brigham (MGB) scope of care. This position requires strong communication and interpersonal skills, as well as the ability to work collaboratively with internal staff and external referral sources.

Key Responsibilities:

  • Evaluate patients and determine their appropriateness for admission to post-acute services, ensuring compliance with local, state, and federal regulations.
  • Conduct on-site clinical assessments to determine patient suitability for admission.
  • Obtain and transmit patient assessment information to appropriate systems, including 4Next.
  • Develop and maintain collaborative relationships with internal staff and external referral sources to ensure customer satisfaction.
  • Nurture referral source relationships to maintain customer satisfaction and identify opportunities for growth.
  • Support challenging transfers using MGB clinical and supervisory resources.
  • Qualify patients for admission, ensuring adherence to MGB criteria.
  • Communicate pertinent information to current medical providers to meet patient and provider needs.
  • Identify and communicate MGB referral influences to the manager for proper follow-up.
  • Consider discharge plan alternatives to determine the most appropriate plan for patients' needs.
  • Achieve payor mix results and account/territory referral and admissions targets.
  • Share educational information and updates with referral sources to ensure understanding of MGB post-acute services benefits.
  • Complete additional department, organization, or network activities per established objectives.

Qualifications and Experience:

  • Graduate of an accredited RN, PT, OT, or SLP program.
  • Minimum 2 years of acute hospital or extended care facility experience.
  • Current license in Massachusetts as RN, Physical Therapist, Occupational Therapist, or Speech Language Pathologist.
  • Knowledge of switchboard practices, paging procedures, and equipment.
  • Computer proficiency, with Microsoft Office applications preferred.
  • Effective verbal, electronic, and written communication skills in English.
  • Broad knowledge of various insurances and government reimbursements desirable.
  • Sales and/or marketing experience preferred.
  • Highly developed communication and interpersonal skills, working with diverse populations.
  • Ability to work independently, be self-directed, and contribute as a member of a team.
  • Anticipates challenges and develops strategies for addressing them.
  • High level of service delivery, with ability to prioritize work, meet deadlines, and adapt to changing situations.
  • Attention to detail.
