Job SummaryWe are seeking a skilled Hospice Clinical Liaison to join our team. As a key member of our Provider Relations department, you will be responsible for building and maintaining relationships with select Boston academic hospitals and surrounding healthcare providers. Your primary goal will be to deepen and expand our referral base, ensuring seamless communication and continuity of care between all settings.
Key Responsibilities- Proactively establish and maintain strong relationships with healthcare providers and facilities
- Coordinate and facilitate communication between Care Dimensions and referral sources
- Provide clinical patient assessment and ensure continuity of care
- Educate healthcare professionals, patients, and families about the benefits and appropriateness of hospice care
- Collaborate with the Director of Provider Relations to achieve departmental goals
- LPN/Registered Nurse, currently licensed in Massachusetts in good standing
- Excellent customer service, organization, and problem-solving skills
- Ability to build and foster relationships
- Demonstrated ability to assess and respond to patient and family needs
- Ability to travel within assigned territory
- Competitive pay range: $45-$50/hour plus incentives
