Department: MHS Care Management

Employment Type: Full Time, 1.0, 80.00 hours per pay period (2 weeks)

Work Schedule: Days

Compensation: $25.57 / hour, based on relevant experience

Overview:
Under the guidance of professional Care Management or Social Work personnel, this role is responsible for managing transitions of care, conducting socio-economic assessments, and documenting findings to facilitate effective care coordination.

Key Responsibilities:
  • Engages with external agencies, post-acute care facilities, and transportation services to assist clients in accessing necessary medical professionals, insurance providers, and support programs to address their social and clinical requirements.
  • Works collaboratively with care management and behavioral health teams to ensure efficient outreach, care coordination, and seamless transitions throughout the care continuum. Provides educational resources to patients in accordance with established protocols.
  • Conducts structured phone communications and utilizes systems to gather patient information for the clinical care team, ensuring that pertinent data is relayed for placement or transfer. Retrieves medical records, imaging, diagnostic results, and other necessary information for treatment teams.
  • Organizes care conferences as required and supports multidisciplinary team meetings for both inpatient and outpatient settings. Coordinates with internal and external resources for placement, discharge planning, case management, behavioral health, and follow-up care.
  • Demonstrates critical thinking, sound judgment, and the ability to operate independently in a fast-paced environment.
Qualifications:
  • Minimum Required Education: High School diploma or GED.
    Required Licensure/Certification: None required.
    Minimum Experience: None required. Preferred: 1 year relevant experience.
