Position Overview:
We are currently seeking a dedicated and detail-oriented Associate Patient Care Coordinator to become a part of our team.

This role is essential for ensuring the completion of established processes and protocols while working collaboratively with all members of the care team to uphold the organization's vision and mission.

The Associate Patient Care Coordinator will deliver exceptional customer service and adhere to Lean processes, supporting the team in achieving financial, clinical, and service objectives.


Key Responsibilities:
  • Gather and maintain accurate patient information, including personal and insurance details.
  • Respond to telephone inquiries promptly and courteously.
  • Conduct insurance verification during scheduling.
  • Confirm and arrange appointments efficiently.
  • Facilitate follow-up services for patients, including scheduling specialist visits and obtaining lab results.
  • Ensure patient documentation is completed accurately, including signatures and necessary uploads.
  • Maintain organized medical records and rectify registration errors.
  • Collaborate with team members to meet specific goals and maintain compliance with administrative policies.
  • Stay informed on current medical requirements relevant to the role, ensuring patient confidentiality and adherence to HIPAA and OSHA standards.
  • Communicate effectively with patients, families, and healthcare professionals.

Work Schedule:
This position typically operates Monday through Friday, within a 40-hour work week, during standard business hours.

Qualifications:
Required:
  • High School Diploma or GED.
  • Minimum of 6 months of customer service or healthcare experience.
  • Intermediate computer skills, including proficiency in MS Word, Excel, and Outlook.
Preferred:
  • 1+ years of experience in a medical front office role.
  • Familiarity with EMR software.
  • Bilingual proficiency in Spanish and English.

  • Strong organizational and time management capabilities.
  • Excellent verbal and written communication skills.
