Compensation: $25.57 / hour, based on relevant experience

Overview:
Under the guidance of professional Care Management or Social Work personnel, the Care Coordinator facilitates transitions of care, performs and documents socio-economic assessments to enhance care coordination.

Key Responsibilities:
  • Engages with external agencies, post-acute care facilities, transportation services, and other stakeholders as necessary. Aids clients and patients in connecting with suitable healthcare providers, insurance companies, payer programs, and additional resources to address their social and clinical requirements.
  • Works collaboratively with care management and behavioral health teams to ensure effective outreach, care coordination, and transitions across the care continuum. Provides referrals and educational materials to patients in accordance with established protocols.
  • Conducts structured phone communications and utilizes systems to gather patient information for the clinical care team, forwarding necessary details for placement or transfer. Collects medical records, imaging, diagnostic results, and other requests for treatment teams.
  • Organizes care conferences as required. Supports multidisciplinary team meetings for both inpatient and outpatient settings as assigned. Collaborates with internal and external resources to facilitate placement, discharge planning, case management, behavioral health, and follow-up care.
  • Demonstrates critical thinking, sound judgment, and the ability to operate with minimal supervision. Must thrive in a fast-paced environment and take appropriate actions.
Qualifications:
  • Minimum Required Education: High School diploma or GED.
    Required Licensure/Certification: None required.
    Minimum Experience: None required. Preferred: 1 year relevant experience.
