Position Overview

The Sparc Network is on the lookout for several Qualified Professionals (QPs) to join our team as Integrated Care Coordinators. Our organization is committed to the vision of supporting individuals in avoiding institutional care, providing essential behavioral health services to children, families, and adults throughout various regions.

We are in search of passionate Tailored Care Coordinators for our Targeted Care Management (TCM) Program.

Qualifications & Experience:
  • A minimum of a bachelor’s degree in health-related fields such as psychology, sociology, social work, nursing, or other relevant human services disciplines, or licensure as a registered nurse (RN); and
  • At least two years of experience working directly with individuals facing behavioral health challenges.
  • Preferred experience includes working with both adults and children with mental health and/or substance use disorder needs.
Key Responsibilities:

The primary role of the Integrated Care Coordinators involves delivering comprehensive, person-centered care management services to members enrolled in Tailored Plans.

Care Management Services Include:

The following tasks will be performed by the Integrated Care Coordinators through both in-person and telephonic support:

  1. Comprehensive Care Management Assessment
  2. Development of Care Plans
  3. Formation of the Care Team
  4. Coordination of Care
  5. 24/7 On-Call Crisis Support (coverage is rotational)
  6. Transitional Care Management
  7. Health Promotion Activities
  8. Medication Monitoring
  9. Addressing Unmet Health-Related Resource Needs
  10. Promoting a System of Care
