As a LPN Home Health Care Specialist, you will work under the direction of a Registered Nurse, providing care that is consistent with our agency's philosophy, policies, and standards of care.
Responsibilities:- Provide high-quality nursing services to patients in their homes, including administering medications, wound care, and vital sign monitoring.
- Work with the interdisciplinary team to develop and implement patient care plans that meet the patient's goals and needs.
- Communicate effectively with patients, caregivers, physicians, and other healthcare professionals to ensure seamless care coordination.
- Assume on-call responsibilities as scheduled, providing 24/7 support to patients and families.
- Graduate of an accredited LPN program.
- Current North Carolina LPN license.
- One year of LPN experience, preferably in an acute care, hospice, LTC, or home health setting.
- Strong computer competencies, including electronic medical record and documentation software.
- Ability to act with integrity, professionalism, and confidentiality.
