About the Role

We are seeking a highly skilled and compassionate Advanced Practice Provider to join our primary care team at Summit Health. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care and working collaboratively with our physicians and staff to provide exceptional patient experiences.

Responsibilities

  • Provide comprehensive primary care services to patients of all ages, including routine check-ups, health assessments, and treatment of acute and chronic conditions
  • Conduct thorough medical histories, perform physical examinations, and order diagnostic tests as needed
  • Develop and implement individualized treatment plans, prescribe medications, and educate patients on self-care and disease management
  • Collaborate with our multidisciplinary team to coordinate patient care and ensure seamless transitions between healthcare settings
  • Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal patient outcomes

Requirements

  • Graduate of an accredited Advanced Practice Program (e.g., Nurse Practitioner or Physician Assistant program)
  • Current licensure and certification as an Advanced Practice Provider in the state of practice
  • Minimum 2 years of experience in primary care or a related field, with a strong track record of delivering high-quality patient care
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound clinical decisions
  • Proficiency in electronic medical records (EMRs) and other healthcare technologies
