Role Summary:

We are seeking a knowledgeable and experienced Clinical Nutrition Specialist to become a vital part of our Home Infusion Pharmacy team, focusing on Total Parenteral Nutrition (TPN) therapy. In this role, you will be tasked with formulating TPN solutions for patients initiating TPN therapy and adjusting existing formulas in collaboration with our clinical pharmacy staff. This position is office-based, emphasizing support and expertise within our pharmacy team.

Work Schedule

  • In-Person
  • Standard workweek, Monday through Friday

Key Responsibilities

  • TPN Formula Creation: Develop and personalize TPN solutions for patients new to TPN therapy, ensuring each formula aligns with the patient's specific nutritional needs.
  • Adjustment of Existing Formulas: Collaborate with clinical pharmacy staff to modify TPN solutions for ongoing patients based on their clinical evaluations and changes in health status.
  • Interdisciplinary Collaboration: Work alongside pharmacists, nurses, and other healthcare professionals to ensure cohesive patient care and optimal nutritional support.
  • Patient Education: Provide training and information to pharmacy staff regarding TPN therapy, ensuring clear communication with patients about their nutritional management.
  • Quality Improvement: Engage in quality assurance initiatives related to TPN therapy to uphold the highest standards of patient care.
  • Record Keeping: Maintain precise and current patient documentation, including TPN orders, adjustments, and clinical notes.

Required Qualifications

  • Bachelor's degree in Nutrition, Dietetics, or a related discipline. Must hold current Registered Dietitian (RD) licensure.
  • A minimum of 3 years of experience in a dietetic role.
  • In-depth knowledge of TPN therapy, including formulation and adjustment processes.
  • Exceptional communication and interpersonal skills for effective collaboration with healthcare teams.
  • Detail-oriented with strong organizational and documentation capabilities.
  • Ability to work autonomously and manage a diverse patient caseload.

Preferred Qualifications

  • Experience with TPN therapy in a home infusion context.
  • Familiarity with clinical standards and guidelines for TPN therapy.
