Role Objective
We are in search of an enthusiastic and skilled Case Manager with a background in assisting individuals who have experienced homelessness and mental health challenges within a supportive housing framework. The Case Manager will play a crucial role in helping clients stabilize their lives, maintain housing security, and achieve their personal, emotional, vocational, educational, and social aspirations. The ideal candidate will possess strong clinical expertise and experience working with individuals affected by mental health issues, substance use, legal matters, and medical conditions. Furthermore, the Case Manager should be dedicated to client advocacy, demonstrate proactive problem-solving abilities, and excel in both collaborative and independent work settings.
Position Details: This is a full-time role with a standard daytime schedule, which may include occasional evening shifts. Flexibility in work hours may be required based on the operational needs of the organization.
- Deliver integrated counseling and case management services that effectively engage clients, promoting overall health, personal development, and housing stability.
- Work collaboratively with clients to create personalized, recovery-oriented service plans that outline both short-term and long-term objectives.
- Employ a harm reduction strategy to support a diverse group of formerly homeless individuals facing mental health and substance use challenges.
- Facilitate referrals and act as a liaison with various care coordination agencies, health and mental health providers, substance use treatment programs, and employment services to secure necessary support.
- Engage clients through regular home visits, community activities, and accompaniment to appointments as needed.
- Develop safety plans for clients and collaborate with the team to de-escalate situations and manage crises effectively.
- Assist in creating and implementing group programs that address the needs of residents and foster an inclusive, respectful community.
- Maintain confidential records for each client, documenting all relevant interventions and assessments, as well as communication with external services.
- Collaborate with the Leasing and Compliance team to ensure ongoing housing stability.
- Participate in staff meetings and individual supervision to uphold best practices in the execution of job responsibilities.
- Engage in a comprehensive training program that includes evidence-based practices to ensure the highest quality of service delivery.
- Perform additional job-related tasks as assigned.

Qualifications and Experience Required:
- Bachelor's degree required, preferably in a related field such as social work, sociology, or psychology; a Master's degree is preferred.
- Minimum of one year of experience working with single adults affected by poverty, chronic homelessness, mental health issues, substance use, and/or medical conditions.
- Ability to work independently as well as collaboratively within an interdisciplinary team.
- Highly organized and capable of managing a diverse caseload of clients with varying needs.
- Able to perform well under pressure, including in crisis situations, and demonstrate flexibility in work scheduling when necessary.
- Proven track record of proactive, strength-based engagement with individuals and families.
- Experience with electronic case management systems is preferred.
- Preferred: Familiarity with evidence-based models, particularly Motivational Interviewing.
- Preferred: Knowledge of populations affected by mental health issues, substance use, or chronic homelessness.
Supervisory Responsibilities: None
Work Environment: This position operates within a supportive housing setting and engages with a population that may experience mental health challenges, substance use issues, disabilities, and other health-related concerns.
Physical Requirements:
While performing the duties of this role, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally, the employee may need to lift and/or move up to 10 pounds.
