Job Summary:

The Discharge Planner will work closely with the case management team to assist in discharge planning services for patients admitted to Westchester Medical Center. As a key member of the multidisciplinary treatment team, the candidate will be responsible for developing and implementing individualized discharge plans in collaboration with patients, families, and community agencies. The ideal candidate will possess excellent communication and organizational skills, with the ability to function independently and prioritize tasks in a fast-paced environment.

Responsibilities:

• Participate in multidisciplinary team rounds to discuss patient care and develop discharge plans.
• Collaborate with treatment team members, patients, and families to identify and address discharge needs.
• Maintain accurate and up-to-date documentation of patient information and discharge plans.
• Work with the case manager and care coordinator to implement the discharge review program and ensure timely issuance of IMM's and discharge notices.
• Utilize computer applications, such as spreadsheets and word processing software, to perform work assignments and maintain patient records.
• Access protected health information (PHI) in accordance with department guidelines and regulations.
• Perform other duties as assigned by the case manager or treatment team leader.
• Establish and maintain effective interpersonal relationships with patients, families, and treatment team members.

Qualifications/Requirements:

• 1 year of experience providing behavioral health services is preferred.
• Knowledge of medical and pharmacology terminology.
• Familiarity with regulatory agency standards, including Centers for Medicare & Medicaid Services and Office of Mental Health guidelines.
• Physical ability to perform duties assigned.
• Bachelor's degree in a human service related field, required.
• Excellent written and communication skills.
• Commitment to enhancing cultural competency.
• Ability to prioritize and work in a fast-paced environment.
• Knowledge of Excel, Word, and Outlook software.
