Discharge Planner

Responsibilities:
- Participate in multidisciplinary team rounds to ensure seamless patient care.
- Collaborate with treatment teams, patients, families, and community agencies to develop and arrange individualized discharge plans.
- Contribute to staff meetings, case conferences, case presentations, and training activities to enhance knowledge and skills.
- Maintain accurate documentation of work activities to ensure compliance with regulatory standards.
- Work with the case manager and care coordinator to implement the discharge review program mandated by New York State Department of Health.
- Utilize computer applications such as spreadsheets, word processing, calendar, email, and database software to perform work assignments.
- Access protected health information (PHI) in accordance with department assignments and guidelines defining levels of access.
- Perform other duties as assigned to support the team.
- Establish and maintain effective interpersonal relationships with patients and treatment team members.
Qualifications/Requirements:
- 1 year experience providing behavioral health services is preferred.
- Knowledge of medical and pharmacology terminology.
- Knowledge of regulatory agency standards; Centers for Medicare & Medicaid Services, Office of Mental Health, and Det Norske Veritas (DNV) standards.
- Physical ability to perform duties assigned.
Education: Bachelor's degree in a human service related field is required.
Licenses / Certifications:
Other: Excellent written and communication skills, commitment to enhancing cultural competency, and ability to prioritize and work in a fast-paced environment are essential.
Knowledge and Skills: Knowledge of Excel, Word, Outlook, and ability to work collaboratively with the treatment team are required.
