Job Summary: We are seeking a skilled Discharge Planner to join our team at Westchester Medical Center. The ideal candidate will work closely with the case management team to assist in discharge planning services for patients admitted to our facility. This is a unique opportunity to be part of a multidisciplinary treatment team and work independently on a professional level.

Responsibilities:

  • Participate in multidisciplinary team rounds to ensure seamless patient care.
  • Collaborate with treatment teams, patients, families, and community agencies to develop and arrange individualized discharge plans.
  • Contribute to staff meetings, case conferences, case presentations, and training activities to enhance knowledge and skills.
  • Maintain accurate documentation of work activities to ensure compliance with regulatory standards.
  • Work with the case manager and care coordinator to implement the discharge review program mandated by New York State Department of Health.
  • Utilize computer applications such as spreadsheets, word processing, calendar, email, and database software to perform work assignments.
  • Access protected health information (PHI) in accordance with department assignments and guidelines defining levels of access.
  • Perform other duties as assigned to support the team.
  • Establish and maintain effective interpersonal relationships with patients and treatment team members.


Qualifications/Requirements:

  • 1 year experience providing behavioral health services is preferred.
  • Knowledge of medical and pharmacology terminology.
  • Knowledge of regulatory agency standards; Centers for Medicare & Medicaid Services, Office of Mental Health, and Det Norske Veritas (DNV) standards.
  • Physical ability to perform duties assigned.


Education: Bachelor's degree in a human service related field is required.

Licenses / Certifications:

Other: Excellent written and communication skills, commitment to enhancing cultural competency, and ability to prioritize and work in a fast-paced environment are essential.

Knowledge and Skills: Knowledge of Excel, Word, Outlook, and ability to work collaboratively with the treatment team are required.
