Position Summary:

The Greater Philadelphia Urban Affairs is seeking a dedicated Housing Case Manager for its service operations. Under the guidance of the Assistant Director of Client Services, the Housing Case Manager will play a pivotal role in ensuring adherence to the standards set forth by relevant housing authorities. This role involves enrolling and orienting clients while managing a caseload of up to 32 individuals. The Housing Case Manager will facilitate connections to essential services, including but not limited to, public assistance programs, educational opportunities, medical care, and ongoing support.

Key Responsibilities:

  • Collaborate with the Case Management team to conduct orientations and registrations for new intakes.
  • Deliver comprehensive housing case management services to an assigned caseload.
  • Perform client assessments, coordinate appointments, and maintain accurate records and case notes.
  • Develop and manage client documentation, including service plans and confidentiality agreements, in compliance with funding requirements.
  • Establish and nurture referral networks with service providers.
  • Coordinate appointments for public assistance, Social Security, and other necessary services to ensure uninterrupted benefit access.
  • Organize medical evaluations and outpatient treatment referrals.
  • Identify and recommend social service resources for clients.
  • Facilitate referrals to educational and vocational training programs.
  • Participate in weekly team meetings and monthly progress review sessions.
  • Engage in training sessions mandated by compliance standards.
  • Contribute to outreach initiatives that support agency programs.
  • Attend all required staff meetings and functions.
  • Perform additional duties as assigned.

Qualifications:

  • Exceptional verbal and written communication skills.
  • Strong organizational abilities with a capacity to manage multiple tasks.
  • Ability to work effectively in a team-oriented environment.
  • Proficient in basic typing and familiar with Microsoft Office applications.
  • Willingness to accommodate flexible working hours, including evenings and weekends.

Education and Experience:

  • Bachelor's degree in Social Work or a related field.
  • 2-3 years of experience in case management is essential.
  • Experience with diverse populations, particularly those facing homelessness, addiction, mental health challenges, or other vulnerabilities, is preferred.
