As a key member of our care management team, you will play a vital role in ensuring the seamless transition of patients from hospital to home, while promoting optimal patient outcomes and satisfaction.

Key Responsibilities
  • Conduct thorough patient evaluations to identify post-hospital needs and develop personalized transition of care plans
  • Collaborate with the interdisciplinary team to ensure efficient and cost-effective care, while promoting patient-centered care coordination
  • Communicate daily with the healthcare team to facilitate high-quality patient progression and transitions plans
  • Evaluate patient readmission risk scores and coordinate readmission mitigation interventions
  • Provide education to nurses, physicians, and the interdisciplinary team on issues related to utilization of resources, medical necessity, and care coordination
Requirements
  • ADN, RN with a minimum of 2 years of hospital nursing experience
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
  • Knowledge of post-hospital care and services available to patients, including home health, infusion services, durable medical equipment, and more
