About the Compensation:

The estimated annual salary for this role is $110,000 - $160,000, based on experience and qualifications. Key Responsibilities:

• Coordinate care for patients with diabetes, including referrals to community-based, ambulatory, and inpatient DSME and support services
• Develop individualized plans of care, incorporating educational goals, learning objectives, and teaching methods
• Provide comprehensive DSME services, including patient assessments, documentation, and evaluation of service effectiveness
• Collaborate with multidisciplinary teams to optimize patient diabetes self-management and achieve better health outcomes
