Job Overview

We are seeking a skilled Nursing Care Specialist to join our team at Lifetime Care. As a key member of our care team, you will play a vital role in creating and managing personalized care plans for individuals with Intellectual Disabilities.

About the Role

In this position, you will work closely with other care team members to assess, plan, implement, or evaluate patient care plans. You will be responsible for monitoring the health and general wellbeing of patients who reside in their own homes, documenting any changes in patient conditions, and reporting on changes to supervisors.

Key Responsibilities
  • Coordinate with other care team members to develop and implement comprehensive patient care plans.
  • Monitor the health and wellbeing of patients, identifying any potential issues or concerns.
  • Document all patient interactions, including observations, treatments, and outcomes.
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • Prior experience as a Registered Nurse, preferably in a similar setting.
  • Previous experience working with individuals with Intellectual Disabilities is highly desirable.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients and colleagues.
  • Valid driver's license and First Aid/CPR certification.
Compensation and Benefits

The estimated salary for this role is $85,000 - $105,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including flexible scheduling, professional development opportunities, and a supportive work environment.
