Physical Therapist 1, Jackson West Medical Center, Per Diem

We are seeking a skilled Physical Therapist 1 to join our team at Jackson West Medical Center. As a Physical Therapist 1, you will be responsible for administering physical therapy treatments in a rehabilitation department, following physician prescriptions and combining professional therapeutic skills with knowledge of fundamental theory and objectives of rehabilitation.
Key Responsibilities- Evaluate and treat each patient according to their specific needs, considering diagnosis, age, functional status, and previous medical history.
- Establish and implement an effective physical therapy treatment plan of care and modify plan as patient status changes.
- Document patient evaluation, daily progress notes, patient care plans, and weekly progress notes in the medical chart.
- Participate as an active rehabilitation team member, attending rounds and family conferences, and interacting and reporting in a professional, organized manner.
- Accurately measure patients, order and perform check-out of wheelchairs and other physical therapy equipment for patient use in a timely and cost-conscious manner.
- Prepare and instruct patients in written home physical therapy programs, develop age-specific patient education materials, and/or classes for patients and staff as required.
- Assist in overseeing and coordinating the learning experience of physical therapy students during their internships and education practicum's with the C.C.C.E., and Senior Physical Therapist of the unit.
- Valid Florida license as a Physical Therapist required.
- American Heart Association Basic Life Support (BLS) and any additional applicable life support certification for Healthcare Providers is required upon hire with at least 6 months validity and maintenance at JHS for the duration of employment.
- 3 to 5 years of related experience.
- Bachelor's degree in Physical Therapy and completion of a required internship program are required.
Job function requires extended standing or walking. Must be able to carry objects weighing 21 to 50 pounds. Able to lift or move objects weighing 51 to 100 pounds with the use of appropriate devices. Jobs in this group require frequent visual acuity to perform activities related to extended use of computers.
