Job Summary

We are seeking a skilled Physical Therapist to join our team at West Gables Rehabilitation Hospital. As a Physical Therapist, you will play a critical role in the rehabilitation process, working closely with patients to develop and implement effective treatment plans.

Responsibilities
  • Evaluate patients and develop comprehensive treatment plans in accordance with physician direction.
  • Provide ongoing evaluation and modification of treatment plans to ensure timely progression and optimal patient outcomes.
  • Develop and implement home programs to facilitate safe and effective discharge.
  • Collaborate with other clinical/therapeutic services and community resources to ensure seamless care coordination.
  • Identify and manage various levels of care necessary to ensure care plan delivery.
Requirements
  • Current state licensure or eligibility for state licensure as a Physical Therapist.
  • Certified BLS or completion within the first 90 days of employment.
  • One or more years of clinical experience and a valid state Driver's License for Home Care requirements.
