Registered Nurse, Patient Care Coordinator - Facilities Team

Position Overview:


The Registered Nurse, Patient Care Coordinator plays a vital role in evaluating and addressing the needs of patients and their families. This position involves collaborating with the Interdisciplinary Group (IDG) to create and implement comprehensive care plans while delivering compassionate palliative and supportive care.


Qualifications:
  • Associate's degree in nursing is required; a Bachelor of Science in Nursing (BSN) is preferred.
  • Must be a graduate of an accredited nursing program.
  • Possession of a valid Florida Registered Nursing license.
  • CHPN Certification is desirable.
  • A valid Florida Driver's License with a clean driving record is required.
  • Automobile insurance and a clear Level 2 Background Check are mandatory.

Key Responsibilities:
  • Provide reassurance and support to patients and families over the phone, assisting them in resolving their inquiries and coordinating in-person visits when necessary.
  • Help patients and families achieve their desired comfort levels.
  • Utilize appropriate resources to address complex situations effectively.
  • Document all patient and family interactions in the Electronic Medical Record (EMR) and maintain communication with the IDG.

Experience:
A minimum of one year of nursing experience is strongly preferred.

Core Duties:

Deliver high-quality, individualized nursing services to patients, including both direct and indirect care. Work under the supervision of the Care Manager or designated personnel while exercising independent judgment in applying advanced nursing knowledge and skills.


Assessment:
Collect and analyze patient data, complete necessary documentation, and prioritize patient needs. Identify issues that require further referral or follow-up and interpret diagnostic results accurately.

Care Planning:

Develop and adjust care plans in collaboration with patients and families, incorporating interdisciplinary support services as needed.


Care Implementation:
Ensure that care provided meets established standards and protocols. Administer treatments and procedures as prescribed, demonstrating an understanding of the unique needs of diverse patient populations. Provide education to patients and families regarding discharge procedures and emergency situations.

Evaluation:
Document care delivered and any deviations from the plan, providing rationale for changes. Monitor patient progress and identify potential complications.

Leadership:
Delegate nursing responsibilities appropriately and integrate cost-effective practices into patient care. Participate actively in quality improvement initiatives and adhere to policies that ensure safe working conditions.

Compliance and Quality:
Maintain compliance with organizational standards and regulatory requirements. Incorporate quality measures into all aspects of care delivery, as evidenced by regular audits and evaluations.

Represent Chapters Health System positively and professionally in all interactions. Uphold the values of integrity and professionalism in every aspect of your role.

Complete all responsibilities thoroughly and utilize sound judgment in your work. Seek guidance from supervisors when needed and assist with special projects as assigned.
