About the Role

We are seeking an experienced Registered Nurse to join our team as an Ambulatory RN Care Coordinator. This role will play a critical part in our efforts to provide high-quality, patient-centered care to individuals with lung cancer.

Key Responsibilities
  • Coordinate care for patients under lung cancer surveillance and those with early-stage lung cancer
  • Utilize Epic, Nuance Follow Up Manager, and Excel to track patients in our programs
  • Provide education and support to patients and their families
  • Collaborate with the care team to ensure seamless transitions of care
  • Advocate for patients to ensure delivery of quality, efficient, and cost-effective care
Requirements
  • Minimum of 3 years of clinical nursing experience
  • Experience in patient education and chronic disease management
  • Home health, skilled nursing, or medical surgical nursing experience preferred
  • Care coordination experience preferred
  • Registered Nurse License in the State of Florida
