Position OverviewLee Health is in search of a skilled Registered Nurse to fulfill the role of Nurse Coordinator at the Advanced Heart Failure Clinic. The ideal candidate will possess a minimum of three years of experience in cardiology. This position is pivotal in guiding patients through their healthcare journey, emphasizing education, resource availability, and effective communication.
Key ResponsibilitiesThe Nurse Coordinator will:
- Oversee patient care from diagnosis through discharge.
- Utilize a multidisciplinary approach to serve as a liaison among patients, healthcare staff, and referring physicians.
- Assess educational needs and develop relevant training programs.
- Facilitate and evaluate educational activities for patients and families.
- Act in various roles including clinician, facilitator, advocate, consultant, and researcher.
- Assist patients and families in navigating the complexities of the healthcare system, aiming to remove barriers that may hinder care and outcomes.
- Coordinate efforts among the medical team to ensure continuity of care and enhance patient satisfaction.
Degree: Bachelor's in Nursing (Required)
ExperienceMinimum Experience: 3 Years in Cardiology (Required)
LicensureRequired License: Registered Nurse License in Florida
CertificationsRequired Certification: BLS
Additional SkillsMembership in professional organizations is preferred. Basic proficiency in computer applications such as MS Windows, MS Word, MS PowerPoint, and MS Excel is essential.
