About the Role

HCA Virginia Healthcare at Home is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a PRN Home Health LPN. As a member of our team, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

Responsibilities

  • Administer skilled nursing care to patients in their homes, working in accordance with physician orders and the Plan of Care.
  • Identify the physical, psychosocial, and environmental needs of patients and provide care accordingly.
  • Verify the Plan of Care prior to each visit and provide care according to physician orders, assessment data, established standards, and guidelines.
  • Document all findings, communications, and interventions, including patient response.
  • Assess patient/caregiver knowledge of disease process, treatment regimen, and/or medication administration, and provide basic and specialized teaching as ordered in the Plan of Care.

Requirements

  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure.
  • Valid driver's license.
  • Vocational school graduate.
