Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Bradford Place Care Center, a leading provider of exceptional living centers. As an LPN, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our organization.

Key Responsibilities
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Conduct daily rounds to monitor patient progress, identify potential issues, and implement corrective actions.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Supervise and mentor nursing staff to ensure they are equipped to provide exceptional care.
Requirements
  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
  • At least one year of experience in a long-term care or geriatric setting.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
