About the Role

The Health Navigator plays a critical role in making healthcare easier for doctors and patients. You will develop trusted relationships with our patients and providers, assisting with various tasks to enable doctors to focus on delivering high-quality medical care. As a Health Navigator, you will work closely with clinic staff, meeting with and calling patients, entering data, and troubleshooting challenges with insurance coverage.

Key Responsibilities
  • Develop strong relationships with patients to assist them with their care
  • Meet with patients during the check-in process, entering data into our software platform
  • Call patients to schedule office visits or remind them to pick up medication
  • Order and schedule procedures, tests, and screenings
  • Identify and track progress on important gaps in patient care
  • Maintain a record of patient interactions and communicate with providers using electronic health records
  • Educate patients on their healthcare options, insurance benefits, and common medical conditions
  • Help patients access community resources and coordinate follow-up care after hospital discharge
