Job Summary

We are seeking a highly skilled Clinical Manager to join our team at CenterWell Home Health. As a Clinical Manager, you will be responsible for overseeing the delivery of high-quality patient care services, coordinating clinical operations, and ensuring compliance with regulatory requirements.

Key Responsibilities
  • Coordinate and oversee all direct care patient services provided by clinical personnel
  • Develop, plan, implement, analyze, and organize clinical operations for a specific location
  • Conduct assessments and reassessments of patients, including updating of care plans and interpreting patient needs
  • Manage the assignment of caregivers and oversee the delivery of care to all patients served by the location
  • Review patient information and determine home health or hospice needs
  • Instruct and guide clinicians to promote effective performance and delivery of quality home care services
  • Assist clinicians in establishing immediate and long-term therapeutic goals and developing patient Plan of Care (POC)
  • Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers
  • Coordinate communication between team members, attending physicians, and caregivers to ensure appropriateness of care and outcome planning
  • Carry out supervisory responsibilities in accordance with Company policies and procedures
Requirements
  • Graduate of an accredited School of Nursing
  • Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment
