The Youth Support Coordinator operates under the guidance of the Intervention Supervisor. This role is essential for the organization and provision of support and intervention services aimed at young individuals who have encountered or observed violence.
The Youth Support Coordinator is responsible for assessing the requirements of youth and their families, connecting them with community resources to fulfill those needs.
This position involves regular meetings with youth and families in various community environments, such as educational institutions or their residences. The Coordinator collects pertinent information from youth and families through screenings, evaluations, and discussions with relevant parties.
In collaboration with the youth, family members, and Clinician, this role aids in formulating objectives for a Personalized Action Plan. Monitoring progress towards these objectives, as well as planning and executing discharge strategies, is a key expectation. Youth Support Coordinators will collaborate within a multidisciplinary team.
QualificationsThe ideal candidate will hold a Bachelor's Degree in Human Services, Counseling, Social Work, Psychology, or a related discipline, along with a minimum of three years of experience working with at-risk youth and families. Bilingual and bicultural applicants are strongly encouraged to apply.
