Key Responsibilities
  • Manage discharge planning for assigned patients, ensuring timely and effective transitions to post-acute care.
  • Collaborate with interdisciplinary teams to communicate discharge planning status and provide education on post-acute options.
  • Conduct psychosocial assessments and interventions to support patients and families in adhering to treatment plans.
  • Advocate for patients and families, addressing social, psychological, financial, and behavioral barriers to successful medical outcomes.
  • Provide counseling and intervention related to treatment decisions and end-of-life issues.
  • Support patient and family empowerment and independence in making autonomous healthcare decisions.
Requirements
  • Master's degree in Social Work (MSW) or Master's of Science in Social Administration (MSSA).
  • Current state licensure as a Social Worker (LSW, LISW, or LISW-S).
  • Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • Preferred qualifications include three years of experience in healthcare systems, hospital social work experience, and knowledge of community resources.
