We are seeking a skilled Physician to join our Hospice and Palliative Care team. As a Physician, you will provide top-notch care and patient experience, building your patient panel and collaborating with knowledgeable Physicians, Medical Assistants, and PSRs.
Your Responsibilities- Provide medical care and services to patients in various settings, including inpatient hospice units, inpatient hospitals, and home settings.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Conduct home visits and provide medical care to patients in their homes.
- Participate in palliative care consultative services and provide pain management expertise.
- Fellowship trained and Board Certified in Hospice/Palliative Care.
- Have or able to obtain a CO Physician license.
- Minimum of two years of experience working in a defined Palliative Care or Hospice setting.
- Experience with pain management a plus.
- Bi-Lingual, strongly preferred.
