Key Responsibilities- Lead a multi-disciplinary team of healthcare providers to deliver high-quality palliative care services
- Develop and implement care plans that address the physical, emotional, and social needs of patients and their families
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
- Participate in quality improvement initiatives to enhance the delivery of palliative care services
