About the Role

We are seeking a highly skilled Care Coordinator to join our Crisis Recovery Unit team at Southwest Behavioral & Health Services. As a key member of our team, you will play a vital role in supporting the most appropriate and least restrictive setting for our consumers, improving their level of functioning, and assisting them and their stakeholders in learning and retaining skills to help them remain in the community.

Key Responsibilities
  • Administrative and Organizational Coordination: Coordinate referrals for admission to the CRU using established criteria, develop discharge plans with consumers, and ensure appropriate levels of care.
  • Collaboration with Outpatient Teams: Work collaboratively with outpatient teams to plan discharge and provide information to clients on behavioral health issues and make appropriate referrals to SBH programs and/or other community agencies as needed.
  • Documentation and Record-Keeping: Maintain accurate, thorough, and current documentation of contact with consumers, treatment progress, and services received/provided, as well as contacts with other agencies.
Requirements
  • Education and Experience: Minimum requirement consists of a high school diploma with 2-3 years of experience in behavioral health, or a Bachelor's degree with 1 year of full-time behavioral health experience.
  • Valid AZ Driver License and Motor Vehicle Clearance: Possess a valid AZ driver license and motor vehicle clearance.
  • Knowledge of Agency Software: Have knowledge of how to access and use agency, network, and RBHA software.
