Pharmacist - PRN Specialist

At City of Hope, we're committed to delivering the cures of tomorrow to those who need them today. As a Pharmacist - PRN, you'll play a vital role in our mission to provide patient-centered care and integrated pharmacy services in a decentralized and centralized practice setting.
Key Responsibilities:- Review and interpret providers' orders for all medications, ensuring therapeutic incompatibilities and cost-effective medication use that complies with the COH Enterprise Formulary.
- Assist with the monitoring of drug therapy, with an emphasis on chemotherapy, and provide and educate on drug information to providers and other clinical staff.
- Oversee technician staff as it relates to sterile intravenous compounding, using centralized and decentralized medication-use systems, including chemotherapy and TPN, medication refilling, and unit dose drug preparations.
- Participate in quality improvement projects with an emphasis on patient safety and engage in multi-disciplinary initiatives as requested.
- Collaborate with medical oncology teams, providing support in treatment plans, oral chemotherapy counseling, and symptom management.
- Act as a liaison between providers and pharmacy, addressing medication orders and resolving drug-related issues promptly and effectively.
- Competently prepare and supervise the preparation of sterile compounded products according to USP 797 and USP 800 guidelines, ensuring safety and quality standards are met.
- Participate in departmental and hospital quality assurance programs, emphasizing patient safety and regulatory compliance.
- Graduate of an ACPE-accredited College of Pharmacy.
- Active license with the State Board of Pharmacy.
- Doctor of Pharmacy or other advanced degree highly preferred.
- Minimum of 2 year's institutional pharmacy experience or strong interest in institutional and oncology pharmacy.
