City of Hope is seeking a highly skilled Pharmacist - PRN to join our team in providing patient-centered care and integrated pharmacy services in a decentralized and centralized practice setting.
Key Responsibilities- Medication Management: Review and interpret providers' orders for all medications, screen for therapeutic incompatibilities, and ensure cost-effective medication use that complies with the COH Enterprise Formulary.
- Drug Therapy Monitoring: Assist with the monitoring of drug therapy, with an emphasis on chemotherapy, and provide and educate on drug information to providers and other clinical staff.
- Sterile Compounding: Competently prepare and supervise the preparation of sterile compounded products according to USP 797 and USP 800 guidelines, ensuring safety and quality standards are met.
- Quality Assurance and Patient Safety: Participate in departmental and hospital quality assurance programs, emphasizing patient safety and regulatory compliance.
- Interdisciplinary Collaboration: Participate in interdisciplinary rounds with medical teams, contributing expertise in medication management and patient care.
- Education and Assessment: Collaborate with the Pharmacy Clinical Coordinator in educating and assessing various stakeholders, including pharmacy staff, providers, clinical staff, patients, caregivers, and pharmacy students.
- Graduate of an ACPE-accredited College of Pharmacy
- Active license with the State Board of Pharmacy
- Doctor of Pharmacy or other advanced degree highly preferred
- Minimum of 2 year's institutional pharmacy experience or strong interest in institutional and oncology pharmacy
