Job Summary:

The Good Samaritan Society is seeking a skilled Nursing Care Coordinator to join our team. As a Clinical Care Leader, you will be responsible for utilizing the nursing process to provide individualized care in the home setting. You will collaborate with inter-professional colleagues to plan, implement, and evaluate care, and document resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations.

Responsibilities:

  • Utilize the nursing process to provide individualized care in the home setting
  • Collaborate with inter-professional colleagues to plan, implement, and evaluate care
  • Document resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations
  • Utilize technology to enhance quality resident care and collaborate with inter-professional team members
  • Delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment, and serve in leadership roles on departmental and organizational task forces and committees

Requirements:

  • Must have effective communication skills to collaborate, negotiate, and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, residents, and families
  • Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations
  • Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure
  • Bachelor's degree in nursing preferred, and a minimum of two years' long-term care or geriatric nursing experience preferred
