Job Title: Attendance Case Manager

CORA Services is seeking a highly skilled Attendance Case Manager to join our team. As a key member of our Community Schools team, you will be responsible for providing case management services and supports to young people and their families who are at risk of truancy.

Key Responsibilities:
  • Develop and implement a plan of service for each child assigned to you, which may include enrollment in services such as Out-of-School Time and provision of resources to attend school regularly.
  • Collaborate with schools, Community School Coordinators, and other School Partners to assess needs and develop strategies to support students and families.
  • Conduct regular home visits to assess families' needs for services and develop a service plan to remove barriers to regular school attendance.
  • Complete weekly contact with families on assigned caseload to support with social services and health navigation as needed.
  • Maintain accurate and timely documentation of services provided to student/family.
Requirements:
  • Bachelor or Master level degree in a Social Services field (e.g. Social Work, Counseling, Criminal Justice or closely related field).
  • Valid driver's license.
  • At least two years' experience in social services; experience in treatment or intervention services preferred.
