Job Summary:
The Attendance Case Manager will work closely with the School District's Community School Coordinator to improve attendance and academic achievement through case management services for at-risk children, youth, and their families. This role requires a strong understanding of case management principles, as well as excellent communication and interpersonal skills.
Key Responsibilities:
- Provide case management services to a caseload of children and youth, including assessments, referrals, and linkages to services.
- Develop and implement individualized plans to improve attendance and academic achievement.
- Collaborate with school staff, community agencies, and family members to ensure comprehensive support for students.
- Document all activities and services provided in case files and database.
- Participate in case conferences, school conferences, and other teaming conferences as needed.
Requirements:
- Completed Bachelor's Degree in Social Work or a related field.
- Minimum of 1-year experience in case management, education, and child welfare.
- Clearances: FBI, Child Abuse, Criminal Background, and Medical Exam & TB Test.
- Valid Driver's License.
