Job Summary

The Lead Medical Respite Coordinator plays a vital role in ensuring the effective care coordination and client support within the Medical Respite Program. This position oversees the Medical Respite Coordinators and Navigators, collaborating with interdisciplinary teams to achieve positive client outcomes.

Key Responsibilities
  • Coordinate medical and social services for clients post-discharge from the hospital and medical respite site.
  • Assist with telehealth appointments for mental and physical health services.
  • Coordinate medication deliveries from local hospitals and pharmacies.
  • Identify and refer clients to local community resources.
  • Provide coordination of food services for patients.
  • Assist with patients' transition to their next housing location.
  • Participate in daily care team meetings.
  • Communicate respectfully with care team members to provide patient-centered care.
  • Act as a patient advocate and liaison between patients/families and community service agencies.
  • Support organizational initiatives to promote a positive workplace culture.
  • Respect patient confidentiality and treat patients with courtesy and respect.
Requirements
  • Bachelor's degree in social work, psychology, sociology, public health, or a related field.
  • Limited License (if applicable).
  • Full License (preferred).
  • At least 2 years' experience in case management, mental health, or a related field.
  • Experience working with underserved populations.
  • Experience identifying, accessing, and referring to local community resources.
  • At least 1 year of supervisor experience (preferred).
Additional Requirements
  • Proficiency in MS Office (Word, Excel, PowerPoint, and Outlook).
  • Ability to learn additional software.
  • Proficiency in general office equipment (PC, printer/fax/copier, telephony system).
  • Experience with electronic health records and scheduling systems.
