About the Role

We are seeking an experienced Registered Nurse to provide community-based care coordination and management for members with chronic and complex health needs in Delaware. Your primary responsibility will be to assess, plan, and implement care that meets the individual needs of our clients while ensuring compliance with regulations and effective communication with healthcare teams.

Key Responsibilities
  • Conduct thorough assessments of patients' medical and social needs

Requirements

To succeed as a Healthcare Coordinator, you must have:

  • A current Registered Nurse license in the state of Delaware
  • At least 2 years of experience in case management or discharge planning
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively within a team environment

Salary: $65,000 - $80,000 per year based on experience.
