Job Summary:
HexaQuEST Health, Inc. is seeking a highly skilled Physical Therapist to join our team. As a key member of our rehabilitation department, you will be responsible for evaluating and treating patients in various settings. Your expertise in physical therapy will enable patients to reach their maximum potential and improve their quality of life.

About the Role:
This full-time position requires a licensed Physical Therapist with experience in acute care, skilled nursing facilities, outpatient, and/or sub-acute settings. You will work closely with multidisciplinary teams to develop individualized treatment plans, provide patient education, and conduct thorough assessments. The ideal candidate will possess excellent communication skills, empathy, and a passion for delivering exceptional patient care.

Key Responsibilities:

  • Evaluate and treat patients with a wide range of conditions, including musculoskeletal, neurological, and cardiovascular disorders.
  • Develop and implement evidence-based treatment plans, incorporating conventional therapeutic techniques and adapting them to meet the unique needs of each patient.
  • Provide patient education and training on post-discharge management, ensuring patients and caregivers understand their roles and responsibilities.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.

Requirements:
To be considered for this role, you must hold a graduate degree from an American Physical Therapy Association (APTA) accredited program and maintain current licensure as a Physical Therapist in California. BLS certification is also required. Experience in clinical specialties, such as orthopedics or neurology, is preferred. A minimum of 1 year of experience in acute care, skilled nursing facilities, outpatient, and/or sub-acute settings is necessary. A strong background in performance improvement, infection control, safety, and education programs is essential.

Compensation and Benefits:
